Information for Referring Providers and Partners

Seek & Find Balance Behavioral Health welcome any referral for clients eighteen and older with behavioral health issues such as Anxiety, Depression, Mood Disorder, Insomnia, PTSD, OCD, ADHD, Bipolar and Schizophrenia. Providing the best possible care for our clients is crucial. If you would like to refer a client, please fill out the form below. Please note: we recommend discharge coordinators refer patients no more than 7 days prior to their intended discharge date.
